The No Surprises Act gives you the right to receive a “Good Faith Estimate” prior to initiating services with me. Under this law, health care providers need to give patience who do not have insurance or are not using insurance an estimate of the expected charges for medical services, including psychotherapy services.

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services.

You can ask your health care provider, or any other provider you choose, for a Good Faith Estimate before you schedule a service.

If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

Make sure to save a copy or picture of your Good Faith Estimate. For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises.

Forensic and consultation (for professionals) services have a different fee schedule. Please reach out if you are a professional seeking these services.

Cancellations

Our cancellation policy requires 24 hours notice for cancelling an appointment. If a late cancellation (less than 24 hours, barring emergencies) occurs, you are responsible for the full fee.

For psychological testing, a deposit of $100 is required to schedule the intake session and a $260 deposit is required to schedule the testing day. These are not refundable for cancellations but one reschedule is allowed without a second deposit/fee.

Health Insurance

Please note that Harbor Maple Counseling and Psychological Services does not bill insurance directly and our providers are considered out of network with most insurance panels. We will be happy to provide a detailed bill (“superbill”), which you can submit to your insurance for reimbursement. Please contact your insurance company to verify reimbursement rates if you plan on submitting superbills for reimbursement. These sometimes require authorization in advance.

You can move beyond trauma, loss, and life stressors.
